Use a free online tool such as Google Forms or Microsoft Forms to create your feedback form. Share the form via a link in the Zoom chat, on a slide, or as a QR code on workshop materials.
Towards the end of your session, encourage people to follow the links and complete the form.
Best Practices:
- Include a mix of multiple choice or rating scale questions (typically either 1-10 or ‘Very Dissatisfied’, ‘Dissatisfied’, ‘Neutral’, ‘Satisfied’, ‘Very Satisfied’) for ease of completion. The easier the form is to complete, the more likely you are to receive completed forms.
- Include at least one open-ended question to allow participants to share more detailed feedback.
